Anonymous Survey- for a client

Hello, I am a first year trainee working with quite a challenging client at the moment. He has obsessions with particular numbers, this causes him a huge amount of distress. He feels he is the only one with this sort of problem and we have agreed to use a survey method to find out other people's experiences with numbers, e.g. noticing certain ones more often.

It is anonymous and should only take 5 minutes to complete. I would greatly appreciate it if you would be able to, we are going to use the data as part of a behavioural experiment. Sorry I cannot give more details but I want the client to remain anonymous.
