Holden Communication Scale

Long shot query, but does anyone have a copy of the Holden Communication Scale for the Elderly? It's a pretty old scale, but am thinking of using it for screening in my dissertation. Plus it would also endear me to my slightly intimidating supervisor if I could find a copy!
