Two things.
1. Isn't that a serious incident? You could have been seriously injured or killed in two minutes. I hope it is being investigated

I have never heard of someone transferring between programs in the UK or from abroad, I wondered sometimes if it has ever happened. Because the course structures differ it could be quite complicated to arrange in such a way that all the competencies are covered. Maybe it would be viewed differently ...

I think it has the potential to be and may well be important. Do not merely disclose, reflect on the experience so that it answers that question.

I think we ought to distinguish between the legal and practical situation so as to be clear.

If you have met the conditions then you have a contract (offer+acceptance=contract). The remedy for breaching this is the salary for the notice period. Unfortunately that is usually not worth pursuing.

I'm in favour of control of the term consultant as applied to protected titles, with good clear assessment. I find the various guidance and opinion about conditional offers confusing where there is a delay in meeting a condition. A conditional offer is a contract. If the conditions are met it cannot be revoked, this is breach of contract. If they are not met, eg an unsatisfactory reference then it is null...

That's difficult. Given the small number struck off it's hard to avoid a bit of 'guess the case'. Maybe that also explains your caution, which otherwise seemed a bit over the top.
