Case studies for HCPC

Hi!

I am currently in the process of registration with HCPC via mutual recognition rights and, after reviewing my application, the assessors had requested some case studies.
I am not familiar with the design of a case study used in UK, I usually make them following :
1. Reason for referral
2. Significant anamnestic data
3. Case conceptualization
4. Intervention plan
5. Brief description of the sessions
6. Reflective thoughts and supervision

Is there anything I should add/remove?

Hi,

Sounds about right, although would probably depend on the model you are using.

My suggestions would be:

Service context
Reason for referral
Assessment summary
- Presenting problems/symptoms
- Relevant History
- Results of psychometrics/testing
Formulation/case conceptualization
Intervention plan
Summary of intervention
Outcome
Reformulation/Critical Reflection
